In order for your child to participate in any on or off-site church (Pioneer Clubs / GEMS / Cadets / The Rock) related activity (e.g. sporting event, bowling, laser tag, shopping, etc.), a parent/guardian will need to complete the waiver below:

will not hold First Christian Reformed Church, the staff, leaders and volunteers, liable for any injuries sustained by my child during any on or off-site, church/club-related activity during the 2018-2019 school year.
